Welcome to the Client Portal for my two practice sites, where registration, scheduling, and documentation are completed.
Please pay particular attention to which office you're selecting, as I offer Telehealth & Adventure Therapy. In either case, documentation will have slightly different requirements.
In rare cases, we may have to schedule Telehealth rather than Adventure Therapy sessions due to weather or other reasons. I suppose if the internet goes down, we may have to schedule Adventure Therapy sessions as an alternative, but that's not as likely... or feasible for many of you. But, that remains an option.
Remember, if you are billing insurance (not EAP or Cash-Only), then you must complete your registration with Advekit PRIOR to booking a session with me, which can be done here: https://www.advekit.com/therapists/db-palmer?inn=1
Please send a picture of the front and back of your insurance card if you're billing insurance.
NOTE: If you have insurance, you do not pay my Cash Rate, so do not be dismayed by the rates that you see. Your insurance plan sets rates that are independent of my Cash Rates.
